Request to join the Scheme form

If you are making a request to join the Scheme and/or are providing further information following an action or decision, please complete the form below.

Motability’s Customer Support and Oversight Team can consider a request to join the Scheme if you are requesting to be permitted to take an agreement on the Scheme (this applies whether you are a new customer, or you are looking to return to the Scheme following a previous termination).

If you wish to submit a request to join the Scheme to the team, please complete and submit the below form. The circumstances of your case will be examined and the team will advise you of its decision as quickly as possible.
